The Democratic Republic of Congo is facing the largest Ebola outbreak in its history, according to the World Health Organization, as the virus continues to spread rapidly through communities already affected by insecurity, displacement and population movement.

The outbreak, which was formally declared on 15 May 2026, had recorded 3,605 confirmed cases and 1,587 deaths by 30 July. That represents a reported fatality rate of about 44%, underlining the severity of an epidemic that health authorities are struggling to contain.

The scale of the outbreak has now surpassed the previous largest Ebola epidemic recorded in the country between 2018 and 2020. That outbreak produced almost 3,500 recorded cases and claimed the lives of nearly 2,300 people.

The latest figures therefore mark a significant escalation for a disease that has repeatedly challenged health systems across central Africa.

The WHO said the outbreak was continuing to intensify, with sustained transmission accompanied by increases in both reported infections and deaths. During the most recent week covered by its assessment, 567 cases and 296 deaths were recorded, the highest weekly totals of the current outbreak.

The organisation warned that the numbers demonstrated the exceptional speed at which the virus is spreading.

The outbreak is being driven by the Bundibugyo strain of Ebola, a relatively rare form of the virus. Unlike some other Ebola strains, there is currently no approved vaccine specifically protecting against Bundibugyo Ebola and no recognised treatment specifically approved for the strain.

That absence of established medical tools is creating another major challenge for health authorities attempting to control transmission.

Researchers, however, are working to develop vaccines. Teams in the United Kingdom and Singapore are among those involved in efforts to produce vaccines that could eventually provide protection against the strain.

Ebola is a highly dangerous viral disease that can spread through direct contact with infected bodily fluids. Symptoms can initially resemble other illnesses, making early recognition difficult, particularly in regions where diseases such as malaria and other infections are common.

As the disease progresses, patients can develop severe symptoms and complications, with death occurring in a significant proportion of untreated cases.

Across Africa, Ebola has killed around 15,000 people over the past five decades, making it one of the continent’s most serious recurring infectious disease threats.

The current outbreak is concentrated primarily in Ituri province in northeastern DR Congo. The province borders Uganda and lies close to South Sudan, creating additional concerns about the possibility of the virus crossing international borders.

The WHO has repeatedly stressed that the geographic location of the outbreak makes regional coordination essential.

Ituri is not the only area affected. Cases have also been reported in North Kivu and South Kivu, provinces that have experienced years of armed conflict and political instability.

The security situation presents a serious obstacle to medical teams trying to reach affected communities.

Large parts of North and South Kivu are controlled by the Rwanda-backed M23 armed group, while fighting and displacement have disrupted normal access to healthcare in many areas.

For an Ebola response, where speed is critical, insecurity can have devastating consequences. Health workers need to identify cases quickly, isolate patients, trace people who may have been exposed and monitor contacts. They also need reliable access to communities to provide information and medical assistance.

When conflict prevents teams from reaching people, infections can remain undetected for longer periods, giving the virus additional opportunities to spread.

Population displacement creates another layer of difficulty. People fleeing violence may move between communities or cross borders, sometimes without knowing whether they have been exposed to Ebola.

The movement of people can therefore make contact tracing significantly more complicated.

The WHO has warned that insecurity, displacement and cross-border movement are collectively increasing the risk of further geographical spread.

The situation also places enormous pressure on local health facilities. Hospitals and treatment centres must deal with Ebola patients while continuing to provide essential services for people suffering from other diseases and emergencies.

The challenge is particularly severe in communities where healthcare resources were already limited before the outbreak began.

Ebola responses also depend heavily on public trust. Health authorities must persuade communities to report symptoms, cooperate with contact tracing and accept infection-control measures.

Fear and misinformation can make that work more difficult.

Past Ebola outbreaks have demonstrated how rumours and distrust can undermine containment efforts. Communities affected by conflict may also have deep suspicions of outside organisations, making communication and cooperation particularly important.

The current epidemic therefore represents more than a medical emergency. It is also a humanitarian and security challenge.

The WHO’s warning about the accelerating pace of transmission comes at a critical moment. The latest weekly figures show that the outbreak is not merely continuing but expanding at a rate that has already pushed it beyond the previous record for Ebola infections in the country.

The comparison with the 2018-2020 epidemic is particularly significant. That earlier outbreak was one of the world’s largest Ebola emergencies and required an extensive international response.

Yet the current epidemic has already exceeded it in recorded cases.

The absence of an approved Bundibugyo vaccine makes prevention especially difficult. Vaccination has played an important role in controlling outbreaks caused by other Ebola species, but those existing tools cannot simply be assumed to provide the same protection against Bundibugyo Ebola.

That has increased the importance of surveillance, rapid diagnosis, infection prevention, patient isolation and community engagement.

The vaccine research taking place in Britain and Singapore could therefore become increasingly important if the outbreak continues to grow.

However, developing a vaccine is only one part of the challenge. Once a vaccine candidate becomes available, researchers and health authorities must establish whether it is safe and effective before widespread use.

In the meantime, the immediate priority remains slowing transmission and preventing the virus from reaching new areas.

The geography of northeastern DR Congo makes that task particularly difficult. The region sits near international borders and contains communities affected by conflict and population movement.

Any uncontrolled spread could therefore create additional risks for neighbouring countries, particularly Uganda and South Sudan.

The WHO’s latest assessment highlights the urgency of strengthening the response before the virus gains a larger foothold outside the current outbreak zone.

For people living in affected communities, however, the statistics represent much more than an international health emergency. Each reported death represents a family losing a relative, while every new case creates the possibility of further transmission within households and communities.

The epidemic also demonstrates how infectious diseases can exploit weaknesses created by conflict, poverty and limited healthcare access.

DR Congo has confronted Ebola repeatedly and developed significant experience in responding to outbreaks. Health workers, local communities and international organisations have previously succeeded in bringing Ebola outbreaks under control despite difficult circumstances.

But the present epidemic is testing that experience at an unprecedented scale.

With more than 3,600 confirmed cases already recorded and weekly infections and deaths reaching new highs, health authorities face an increasingly urgent race against time.

The WHO’s warning is clear: sustained transmission, insecurity and population movement are creating conditions in which the outbreak could continue expanding.

For now, the focus remains on containing the virus, protecting healthcare workers and communities, strengthening surveillance and accelerating research into vaccines and treatments for the Bundibugyo strain.

The latest outbreak has already entered the record books as the largest Ebola epidemic in DR Congo’s history. Whether it becomes an even larger regional crisis will depend heavily on how quickly health authorities can overcome the security and logistical barriers standing between them and the virus.
